Quick access toolbar losing items

In my ArcGIS Pro 2.1.3 Advanced product, items I add to the quick access toolbar are not staying. I've had to manually add items to make it look like this:
but sometimes, without consistency I'm aware of, the bar is dropped back to the original 5 items on the lefthand side.
Is there a setting I'm missing, or is this a technical issue?
