That looks like you are part of an organization (eg university, gov't etc), hence, you wouldn't have access to your personal one.
In any event, just contact your regional esri office or try to track down your customer number if you purchased software as an individual
I contacted the local Esri office, but they haven't done anything, and I'm still looking for help!
