GeoForm Search Widget address text to Feature Service

I'm using a GeoForm to capture some data for trash pickups across the city. The data gets sent to a Feature Service on ArcGIS Online, where it is stored. 

I want to also store the address entered by the user in the Search Widget in the 'Select Location' section of the GeoForm (see below). For example, if a user types in "611 Opal Ln, Richardson, TX", then that address (string) should be stored in a separate field of the ArcGIS Online feature service that stores the data. 

What modifications do I need to make to the code (and in which files) in order to accomplish this?
