Createing Shapefile from table (X,Y and X,Y)

The purpose is to create a shapefile from a table with 2 pairs of x,y coordinates.  The code is attached.  The program runs 6 to 7 times and then a run time error appears and the .mxd project
closes or "bombs."  Please take a review. I am open to anu suggestion!
