Information Lookup is broken

Hi,

My existing Information Lookup app is broken, and in attempting to make a new one, that also didn't work.  This is what I found:

Hi Alison,

Can you provide the link to the app that is not working? Also, when you went to create a new application were you able configure the application or did you receive these errors when you went to create the Information Lookup application?
