Image service will not start - error message

I have an image service that consists of one image mosaic dataset. When I try to start the image service I get the following error:

Configuration BLF/BLF2009Mosaic.ImageServer can not be started.
Server Object instance creation failed on machine 'machinename'.

...where 'machinename' is the name of our ArcGIS Server.

I'm not understanding the problem from the error message. What can I do to fix this?

Where is the mosaic dataset stored, file geodatabase or SDE?  If in a file geodatabase, can you confirm the ArcGISSOC account has permissions in the Security tab of the folder the file geodatabase is in.
