When I use Field Maps on my phone to take a point, the initial page is a list of weeds and I’d like it to be the blank form I created/have the option for "survey point". Any idea how to change this?

It looks like those are the templates you can choose from. They appear to be based on the Common name of the plant you are mapping.
If you want to manage templates, you can do that using Field Maps or Map Viewer, however, creating a new feature generally requires selecting a template. Check out this article for more information https://www.esri.com/arcgis-blog/products/arcgis-online/data-management/optimize-templates
