Importing addresses from csv

I am attempting to import addresses from a csv file.  I have attached a sample table.  I have tried all the settings in the add data but none of the addresses get imported and I'm not sure why.  Have anyone else had a problwm with this?

Is this with ArcGIS Desktop or ArcGIS Explorer? Which version? Does the table add but just not display what you expected, or what exactly is going on?
