Surveys list in EXB

I am looking to list all our surveys in a list widget using Map service. I want to display all records when we select list item, provide options to edit existing record, create a new survey, filter and view report for specific record. 

is there any widget to have all these functionalities? do we need develop our own custom widget ? Just wanted to confirm this before starting development

Thanks for asking here. Unfortunately, there is no existing widget with all these functionalities. You will need to develop a custom widget.
