Survey 123 Failed to Submit in Experience Builder Widget

We are running into an issue where people randomly receive a failed to submit for the survey widget in Experience Builder. These people are able to submit to the survey at other times but randomly the survey fails to be submitted with no further information. 

The layer attached to the Survey has all of the editing capabilities turned on and the survey and layer are shared with the appropriate groups and people for creating surveys.

This number can be used to search for and subscribe to the bug on the Esri Support site. If the issue is reported by other customers it will be attached to the same bug report, which helps us assess the impact of the issue and prioritize it accordingly.
