Edit widget showing different attributes than are available in the map data

The edit widget is showing feature attributes that were included in the map data.  The data has been changed but the edit widget isn't reflecting the change.  I've tried clearing my cache, reloading the page, and having a coworker try to edit it.  In the image you can see the legend is reading the data correctly however the edit widget has different data.

The layer's editing template can be different from the layer's symbology. I guess...

That was it, thank you.  I wish they put that information in the configure widgets online or somewhere obvious to find it.

@Allen_Zhang Is correct above, you'll need to update your editing templates for this layer in Map Viewer.
