ArcGIS Dashboard - Date/Time Formatting for URL to Survey123

Hello!

I have a dashboard I built that I am using as a webapp to allow our local hospitals to update their Diversion Status meaning if the hospital is fully open, partially open or closed to ambulances. I've attached an image of the Dashboard. I am using a URL Parameter hyperlink to pass the info off to Survey123. As part of the URL I pass off the Date/Time that the hospital last changed it's status. My URL works perfectly when I launch click it from a map/popup window and hands the date/time off correctly to Survey123 to use. However, for my dashboard I am using a list to display the current statuses and would like to remove the map if possible. When I added my update URL hyperlink to the list it passes off most of the information correctly to Survey123 but it appears the date and time is in the wrong format. I've attached a photo of the dashboard where the update buttons are as well as below how the formatting looks when I click from the popup and the list.

I am hoping someone may know a method for me to be able to launch these surveys from the Update button on the list so I can do away with the map on the dashboard. Thanks in advance for any insight you can provide!
