Since I have updated to the latest version on Survey 123 I am unable to use a form that I created a month ago. It allows me to publish it, but it won't let me summit an entry. When in go to my Survey 123 online i get an error message that says "The survey does not exist or is not accessible. " despite having the thumbnail icon for me to click one.
S123 Reports and Integromat is working for me this morning. I guess whatever happened was fixed.
I can confirm as well. I can now access my survey via the Survey123 site, but generating reports is failing. Which means Integromat is still failing. I haven't had any comments on my support ticket since yesterday afternoon. Hopefully we all get some information today.
The survey123 website using Enterprise appears to be working appropriately for me now as well.
The only thing that is not working for me currently is generating a report. It submits, but nothing generates.
I just checked my surveys and they are back up too!
I was able to open a survey in the survey123 website using Enterprise, can anyone else confirm the bug is fixed?

I just checked my surveys on Portal and we seem to be having the same issue.
Following. This issue just cropped up for me today as well. Surveys were previoulsy working fine. Funny thing is, other surveys that post to the same feature service still work. I've tried updating services, deleting and republishing surveys, and keep getting this same error.
We have also opened a ticket with ESRI.

My issues are also with our Enterprise Portal. Same availability of information as April Chipman mentioned above. I've got a trouble ticket it and will let everyone know if they get back to me.
We are also having the same issue with our surveys on our Enterprise Portal. Everything in AGOL seems to function normally. Is there any indication that it is a widespread issue? I see that we are all posting within the same day.
I've had this problem today as well. Yesterday everything was fine. Today, I can't access my surveys via Survey123 and neither can Integromat. All the data is viewable in Portal, and my field techs can still submit data, Integromat and I just can't deal with it through the website.
Did anyone ever get a solution to this? I haven't done any updates or worked in Survey123 for a few weeks at least and today it decided to stop working. It's 2022, why has this been an issue for 4 years off and on for people?
Resolved: The owner of the FL had been changed. Once the owner of the item was reverted back and the FL was moved to the original survey folder everything worked as normal.

This error appears for me from time to time.
I am creating a survey online. I open it in the connect app so I can add linked content to allow the survey to use a custom map, in this case Property Boundaries, and republish. Everything works fine and people are able to submit surveys no problem.
However, If I want to make a change via the browser to some of the settings such as what can submitters do? I can change the radio button option but when I try to save it says the survey doesn't exist or is not accessible. But it exists! I am in there and it works! 🙂 I will not rebuild this survey yet again to get to that option
Were you able to resolve this issue? I have been experiencing this now and can't wrap my head around what has been the issue. Before this week, the form has been working really well on all platforms - web, desktop and mobile apps.
I am having this issue as well
I am now realizing that my data goes to the feature layer (in portal), but does not talk to S123 website (as I too get the Survey does not exist, but the icons for the surveys are there)...my Survey folders in portal that are autocreated when a form is published in Connect are also empty...I have over 160 records and the data collection on my phone works like a charm...I didn't see a clear answer in the forum for this issue....do I need to move the form and webmap files into the correct folders in portal? or some other work around?
I hope someone can at least answer...I have posted stand alone questions before and have not received an answer for over a month sometimes...
After hours of frustration this Post helped! I found that some staff had our Enterprise Portal selected as the active portal in their settings in Survey123, even though they weren't signed in. Highlighting ArcGIS Online as the active Portal they then were able to see the Public Forms from our ArcGIS Online account.
I had encountered the same issue recently, finally found what was causing the problem for us. Our surveys became available when we got rid of the ampersand (&) out of the form title. Very funny it was the cause but definitely did the trick. Maybe other symbols (eg. /,-,_,= etc) could cause the issue as well. Hopefully this helps debunk the mystery.

You can set the active portal via the Settings page - on the Survey123 home screen, click on the menu button, then settings, then Portals.
Where did you switch the 'active' portal? Survey123?
I'm having this same issue, months after everyone stopped commenting here.
I encountered this same issue today and wanted to provide my solution, as it's not listed here. On both my PC (Windows) and tablet (Android) I was attempting to access a public survey but kept getting the 'does not exist' error. In both cases I found that the last location I had logged into with Survey123 was an ArcGIS Enterprise site; I wasn't currently signed in to the site on either device, but I guess Survey123 was still looking there for the survey, even though it was published to ArcGIS Online. So, I switched the 'active' portal to ArcGIS Online, signed in/out, and the survey was accessible.

Hello. I signed out from Survey 123 and started all over and it fortunately worked. So i really do not know what i did to solve the issue but i advise you try what i did for i know its real aching having continuously that error.

Could you try copying/pasting the URL into a text editor to double check the formatting? Another way to debug would be to record a log file from Survey123 while the link is clicked (the application can be opened previously) - see Troubleshoot—Survey123 for ArcGIS | ArcGIS

On the windows device with Survey123, can you go to "C:\Users\<your username>\ArcGIS\My Surveys" and see if there is a folder whose name is the ID of the survey?
I am experiencing a similar issue to these mentioned above. I can see the survey in the list of "My Surveys" when on a device. I can launch it from within Survey123, but when I attempt to add a hyper link to a the Collector App pop up window and launch the survey from there I get the "Survey not Found...." at the bottom of the app. I can SEE the survey sitting up above in the list (its the one with the leaf icon), but the software says it can't see it. if I click on the survey from the list, it launches and I can fill it out- but I was hoping to have it launch from Collector and have a few fields pre-populate from another layer.
I have checked permissions, settings and everything looks good. I am not sure why I am getting this and am hoping there has been a solution since the other users posted.

The message you received is a generic message; there are a number of different causes that could lead to this being displayed. Can you provide the XLSFile of the form to attempt to replicate?
Does anyone know if this problem got resolved? I have the same issue with a survey that I created almost a year ago and up to date it has over 600 records. I also had ESRI Support looking into this error with no resolution. We got as far as discovering that the forminfo.json file was the issue (although it had no noticeable variation from a working version), but did not get a clean solution. In addition to the solution that Joe shared above for creating a new form from the existing xls, I also discovered that if you create a new survey from the existing survey's feature service that I could then view the data that was already collected from the Survey123 website. This solution would allow you to have a working Survey123 form that uses the existing feature service for data submission and reporting.

I contacted Esri Tech Support, but they were not able to help solve my problem. So, I decided to start over with my Survey123 form. I did find out that I could create a new S123 project with my original form (my Excel spreadsheet XLS file), which saved me a lot of time. I deleted my earlier S123 work from ArcGIS Online (save any survey information first by downloading your existing data) and from my computer (save a copy of your original XLS form somewhere for safe keeping). I opened Survey123 Connect and clicked on the New Survey button.
Choose the "File" option in this New Survey window. Give the survey a name and you have a new base to start from. Make what ever little adjustments are needed and publish your new survey. This worked for me and did not take too much time to do.

I recently uninstalled connect version 2.7 and reinstalled version 2.5. When I re-publish my surveys using 2.5 I still get this error and cannot access the data. When I delete the form's folder in AGOL (form and feature service) ,and delete the info, iteminfo and forminfo json files, and then publish the form again with 2.5, I can access my data. Any thoughts on why I might be experiencing this error? I have a couple of forms that I cannot delete and then republish, but I still need to be able to access the data and produce reports on. Any thoughts on how to fix this error on these forms?

Thank you for your reply. I sent you an email with my xls. My issue is not the same as the issue in your referenced post. I can see the icons for my surveys in survey123.arcgis.com but when I click on the icon to access the data, I get this error message:
This only happens for the surveys that I have published in the past 2 weeks, which roughly corresponds to my installing Connect version 2.7.

No, I haven't modified the .info file. This is happening now to three of my surveys - all of which were published using the latest version of Connect (version 2.7). 2 of those surveys are multi-page and the other is a single page.

Did you recently make any changes in the "My survey designs" folder? I was messing around with the .info file in there to try to get an offline basemap working....I think I made some schema change that somehow broke the survey. After I went back into the .info file and changed it back to the original, I was able to view my survey on survey123.com with no error message. Not sure if it was a fluke or a fix, but it worked for me.
This error appears to impact all of my surveys that have been published with multiple pages.
I am having the same issue with a survey that was recently published using the latest version of connect. I can submit data, view the data in the feature service, but cannot access data through survey123.com. I get the error message "This survey does not exist or is not accessible". I need to be able to analyze this data and export reports. Any suggestions?
I am having this issue as well. I have been editing the survey in connect and tried republishing it. I am able to download and submit the survey on tablets. When I go to survey123.com to download the results, I am not able to access the survey, I get the error "this survey does not exist or is not accessible." I was able to access the survey results yesterday, and now this morning I am not. Any ideas of what could be the issue?

I am using:
I created a brand new, simple survey from Survey123 Connect’s “Advanced” template. I publish the survey, via Survey123 Connect, to my ArcGIS Online (AGOL) account with not warnings or errors. I download the survey to my Survey123 app on Windows 10 and “Collect” some data then submit the data. All of that works as expected.
I am unable to access my survey from the Survey123 website in AGOL. The survey tile on the “My Surveys” page appears, but the tile does not show that any records are present and the ellipses in the bottom right-hand corner is unresponsive. If I click on the survey tile I get “The survey does not exist or is not accessible” message on the web page.
None of the potential solutions discussed earlier in this thread apply to my simple survey. Has anyone else found a resolution to this problem?

We believe to have identified the issue related to the inaccessibility of surveys, and have made an attempt to resolve it. Please try to access your surveys again and let us know the results.
I found I had this issue as well
I narrowed it down to having a field in my survey with
Likert as the apperance > No label in the heading but a hint
Republished survey with a label and it worked
We too are getting this error: "The survey does not exist or is not accessible".
The problem occurs on a recently updated survey that works fine in the mobile app, successfully adding and submitting records. The form and the data are not accessible on the Survey123 website. The hosted feature service is functioning properly and accessible.
This same issue has reoccurred with older surveys based on earlier versions of the same basic survey design. Surveys that have not been modified at all suddently stopped being accessible via the S123 website, perhaps due to website updates.
All of the surveys with issues were designed on a somewhat outdated release of S123 and S123 Connect. The reason we have been holding off on upgrades is because it appears that there is no .msi installer for these products on Windows, which is what our IT department requires for frequent updates for products that are changing quickly like S123.
We could just create a new survey and migrate the 5k records as a workaround, but it sure would be nice to avoid this issue in the future.

I am able to submit surveys under links that I have already sent out. It seems that the management of the survey items is unavailable. I have attached an image of the management panel. All of the options are grayed out.
I have tried to publish one of the ESRI community surveys and I can access those successfully. It seems the issue is limited to surveys I have modified.
I am also experiencing this issue. I publish via Survey123 Connect and all of my surveys are experiencing the same error message. I was using the tool and the surveys successfully hours ago. It is essential that these services are restored. We have a very large project that is occurring on 1/29 that relies completely on these surveys. I have tried the same steps as David and am seeing the same error.
I achieve the same result when I select the option to 'Manage in Survey123 website' from the side panel. It still shows up as not existing.

There is a possibility that the survey you are publishing in Connect, and the one you are attempting to access in your online Survey Gallery may not be the same.
I have tried to submit it via the mobile app as well as the downloaded version on my desktop. In Connect it will let me publish the form with no problem. But it still won't let me submit anything to the Survey.

When you say that it allows you to publish, but not submit, can you please let us know where you are publishing (Survey123 Connect or web designer) and if you are attempting to submit using the Survey123 field app (mobile) or the web form (online)?
