I was hoping to set these two notes side by side. Is that an option with Survey123. Also could I include a checkbox or choice to select one or the other?
You can look into the grid layouts, but I find them restricting at times. If this is the only item in your survey which requires a side by side, you could put your notes into an html table. One example of doing this:
