Matrix Decision - Survey 123

Question

Hi,

I'm wanting to create a form to help me identify plant species. But I need to create these validations based on 10 questions. In other words, you need to create a decision matrix. How can I do this in Survey Connect, I'm new to the tool and I don't know how to use the XLS Form properly.

In the end I need to get an answer, which is the species name. This answer is what this database's filter.

That's not what I meant unfortunately. I need that among the 10 questions at the end there is a filter in this database and inform the species name.
