How was the Local Government data dictionary (Geodatabase Documentation) html page created?  Was it done using X-Ray?

Almost certainly.  There are several different tools out there that produce similar reports.  And just to be clear that would be the X-Ray for Catalog (the one for ArcMap is quite different).  The X-ray tool is handy, but the documentation is sketchy in my opinion.
