orthorectification error 000425

Hi there,
does anybody know about the error 000425 occuring when trying to orthorectify an image with a DEM/ constant elevation value? It states it is impossible to create the outputfile but I assume the problem lies before that: does it have something to do with the rational pyramid coefficients RPC? I don't know how an image can have those..
can anybody help me please?

What are the properties of the input image?  Number of bands, bit depth, etc... What format are you selecting for output?  Does the input have RPC information?

I have found several threads raising this issue, but none has been resolved.

To answer the questions above,
I have tried the "Create Ortho-Corrected Raster Dataset" tool with both single-band and 4-band .tif files - both produce the same 000425 error message.
I am using a DEM to orthocorrect
Bit depth is 16
I am using .tif as the output format.
I have no rpc information
I am working in ArcGIS 10.1 SP1
The error message 000425 appears almost instantaneously as the tool begins to operate.
