Output a report, using a template (customizable)

I would like our survey crew to go collect all their data and pictures and then, back in the office, be able to select a report template that is customizable, that can bring in all the data that was field collected.

If you could let me know what apps I would need to use that would be great.  We'd love a client ready survey report where each page corresponds to a control point.  Items needed on the page would be coordinates, descriptions, pictures

Have a look at Fusion Report Designer of Business Analyst Desktop. Using BA Desktop, you can create custom BDS layers using your own data and then bring them into Fusion Report Designer to create reports.
