Published Map does not show standard esri toolbar

I'm working with ArcControl in vb.net.   (I started with the AttributeQuery sample).  When I run the exe, no toolbars are showing in the published map in ArcReader.   (as in Standard toolbar, or the basic File menu).  If I open ArcReader and then open my published map, all is as usual.

Oops,  the maptools does not show the toolbar either.   Is there a way for me to programmatically load the toolbars in my vb.net program, or do I need to rebuild each tool that I the users need to use?
