How to create a layer from a feature service published by ArcGIS Server to use as input for Add feature Service URL to AXF tool?

In ArcMap, publish your service, enable Feature Access in the service.
Close ArcMap, open ArcCatalog.
Connect to ArcGIS server with a user account, not as admin or publisher.  This is important.
Navigate to your service thru the user account in ArcCatalog, right-click on the Feature Service and select Create Layer.
Give it a name and note the location where is will be stored for use in the Add feature service URL to AXF tool.
