appbuilder application deployment

I have deployed an appbuilder application in IIS

done also proxy configuration

and i need help of how not show sign in arcgis information when i open the application?

And when I activate proxy in config.json file of the app then it shows blank page

   You posted this question in the GeoNet help space again. What Appbuilder are you taking about, Web AppBuilder or Flex App Builder?
