403 - Forbidden: Access is denied

When I deploy a silverlight viewer, the website displays a 403- Forbidden: Access is denied 'You do not have permission to view this directory or page using the credentials that you supplied.' message.  I can edit the website through the application builder on the same server but the deployment doesn't allow me to see the end product.  Any ideas?

• Windows 2008 r2 in a DMZ


• ArcGIS Web Handler


• Self-Signed SSL credentials are established


• Map services are not secure


• Web Handler is token based.
