msd and initial extent

I'm having a strange issue regarding the initial extent for my map service in flex viewer.

When I publish a map service as an MXD, all is as expected.  If I publish as MSD, the extent is NOT how I set it up in ArcMap.  It is zoomed in slightly and the whole map is pushed down enough so that my content is not shown in it's entirety as it should.  The preview of the service shows this skewed extent as well.

I couldn't find a solution to this strange behavior so I tried setting the initial extent in the config.xml and that worked....However, now when I use the zoom or pan tools, it works in an opposite fashion. Panning right goes left and vice versa and zooming to a part of the map takes you to a part on the other side instead.

Does this make sense to anyone??

Yes, I had similar experience with msd file, and ended up using an mxd file instead.

However I also find that sometimes after closing the browser and opening the application again the initial extent is not taken into
consideration and the map is zoomed fully to the lowest tiled scale. After clearing the cache it is ok again.

I still never found out why it acted the way it did when specifying an extent in the config but I did fix my issue.   It turns out the issue apparently comes from ArcMap when saving the project as an MSD, the "Prepare" window pops up (where map errors/warnings are displayed) and once that happens it skews the extent somehow.  All I had to do was zoom to full extent while the Prepare window was open and then save the MSD and the extent was pretty much correct.
