Survey123 Website form doesn't pull attributes into Make (formerly Integromat)

My coworker created a new survey using the Survey123 website, which created the hosted feature service upon publishing.  We would like to send an email to a coworker when a new survey is submitted.  We are using ArcGIS Enterprise 11.1 and the layer is hosted on our Portal.  We are able to connect to the Enterprise without issue and can see our surveys in the drop-down list.

We created a new scenario where Survey123 is watched and when a new record is added, it will send an email to the desired team.  When we click on the subject line or body, we are not given the list of attributes from the survey that I have seen with other surveys.  See the attached image for reference.

Previously, I created two scenarios in Make that used a survey created by Survey123 Connect.  These surveys used the same existing hosted feature service.  The Outlook fields provided the attributes from the survey and it worked without issue.  See image below.

The only work around I have found is to use the feature service to create a new survey in Survey123 Connect.  Make seems to have no issues with those attributes.  I am wondering if Make only works with Survey123 Connect or if we are possibly doing something incorrectly.

In the survey123 website have you checked all the necessary payload options for the webhook? It might just be as simple as attributes missing from the payload.

Yes, we have all options for payload checked.

Although we have not solved our issue as to why that web version survey is unable to pull the attributes into the email, we were able to get it to work with a copy of it.  I had her save her existing form using Save As and the copy worked.  That makes me wonder if her original form is corrupted in some way that didn't carry over to the copy.  We had the same issue with a few test forms we created in the web version while troubleshooting, so I'm not sure if that was truly the problem.  There were no settings that changed between the original and copy versions, so I'm at a loss.  If I am able to successfully recreate it in the future, I will update this ticket with that information.
