Survey123 Form- Error initializing form- and hyperlink list

I am working with ArcGIS Enterprise Portal 11.1 (Development) , and survey123  version 3.18

My goal is in my survey form is to show a hyperlinked images’ list that are stored in my enterprise account.

Here are some screen shots for better understanding of my problem.

Note. everything works beautiful on Survey123 connect, the problem is when the form is published everything gets wonky.

The field app and web app often times act differently with html.

Do users need to answer the divmap question or are you just using it to display the Url?  As an alternative, you could set up the URLs as a series of note fields and have them relevant based on the answer to your imagemap question. 

It appears that divmap is a 1:1 match for imagemap. If so, and users need to answer both you could use the note fields to make a calculation to capture the answers for the divmap question.

Hi Neal,

No, the users do not need to answer the "divmap" question. The "divmap" is to hold the "url' for the clickable image. 
And thank you for the suggestion in setting up the 'urls' as a series of note fields, it did work.

Yes, the 'divmap' is a 1:1 match for the 'imagemap'. Could you show me  the calculation alternative?

I didn't have an alternative for the urls, the calculation I suggested would have been if you you needed an answer for the divmap questions and it was 1:1. There are several ways you could do that.   Have a default answer for each of your relevant note questions and then coalesce into a separate that you would record.
