Survey123 Error: Initialing Form....

Hi.  I'm trying to open a newly created/published form with arcgis.com but I'm getting this error:

   TypeError: Cannot read property 'split' of undefined.

And it shows just my first group header name with back/next buttons

I can open, view and submit data just fine with my mobile device.

Yes, my survey was created with survey123 Connect.

Can you please upload the XLSForm that you used to publish your survey to this thread?  You ma have to click on the 'Use advanced editor' link at the top right of the message box.  Otherwise, you can also send it to barmstrong at esri.com
