I am looking for advise to resolved this issue (see screen shot).
The send error occur when I tried to send my survey.
This appears to be a database level error suggesting that the row (data entry) is too long.
It could be tied to being too large a data entry, misconfigured fields, or missing corrupted fields.
It's not a common Survey123 error. If you need help narrowing it down further we'll need:

We had an ArcGIS Enterprise for Organization account and the ArcGIS Survey 123
is created and stored in this account.
We haven't encountered this issue when we conducted the survey on web app.

Probably not the issue but I have encountered an error where I have to open a separate tab and login to AGOL and then resubmit the survey. It is probably not the same issue you are dealing with but might be worth a shot in the dark.
