Publishing Error 500 Folder Title Not available

I am trying to publish my survey to Enterprise and am running into the publishing Error 500. I created the survey on my Survey 123 connect AGOL account, I then deleted the survey and ensured it was deleted in AGOL and my computer's local files ArcGIS\My Survey Designs.

I have attached the a screen shot of the error and the xls form. I have read through the other posts in the community with solutions on error 500 and have not found a solution that works on my end.

Any help would be very greatly appreciated!

For anyone else that may have this issue, it turns out that I had incorrectly coded one of my text questions as an esrifieldtypedate. Once I cleared that issue I was able to publish!
