I'm wondering if it would be possible to have a checklist and then associated with each item on the list would be either a blank text field for adding a number or a dropdown to assign a rank.
For surveying natural communities, we'd like to be able to select the botanical species we're seeing as we walk through an area and then also assign an estimated abundance metric (1=rare, 2=infrequent, 3=common, 4=abundant, 5=very abundant). So I'd need to have a list of plant species (about 40 - 80 species) with checkboxes/radio buttons that the user selects and then a text field or dropdown next to each identified plant species. This would be preferable over having to select the species in one question and then rank them in a second question.
Is this possible?

You need to use a repeat for this.
I have species inventory forms and they look like this. I also have a check for dups. We have 11 state lists that total over 65,000 species total. I have it so a user can search on Plant code, common name, or scientific name all at the same time.
I have this and other forms for Terrestrial, Wetland, and Lotic systems. We share them out so if you would like a copy of the forms send me a PM.
