I have a survey that I have published and it works great with the iOS app. However, every time I attempt to test it on a browser I get an "Error: Initialing Form..." with no additional information and the only option to click the close button. The form won't expose relevant groups properly and the submit button is non-responsive. This survey does have a lookup table for a pull-data function, but I've checked it for spaces and can't find any that I can tell would be problematic based on reading issues others had. Has anyone else encountered an issue like this? I have tested on Chrome (ver 70), Edge (ver 41) and IE (ver 11) with no success.

I have also verified the issue exists on mobile device's browsers (tested on Safari ver. 12). However, it works without issue on the iOS app and I have verified it on the Android app as well.

The issue is the column '2ndPhone' in the CSV file - the webform does not allow for numbers to begin a column in a CSV file.
