Hi, 

It looked like the message you are seeing was copied into the link for 'My Survey', not the link to the Survey.

I'm now able to access the form.  I'm not seeing the issue.  The issue in the photograph looks like it might have been limited to that load of the page.  Can you try again.  If it continues what browser/device are you using?

I was using chrome browser. I tested a point once but forgot to upload photo evidence. That went through (which it shouldn't have) because photo evidence is a requirement.

So now that I'm attempting to test it out uploading photos (below 10MB) I'm getting the error.

I'll try again with Firefox and see if it happens.
