Error: An error occurred when querying data from the feature layer. 400: UNKNOWN_ERROR Unable to perform query. Please check your parameters.

I have tried to upload my custom template and I get the error message above.

I then downloaded the "create sample template" and attempted to load that back in (with no changes) and received the same error.

Sounds like there is something wrong with your survey. Enterprise or AGO? Connect or web app?

Was this working preciously? If you publish a big update that changes the itemId, you need to refresh the Survey123 Website page before you try to upload a template. That could create the UNKNOWN_ERROR message.

Some troubleshooting steps to try:

  1. I would start by creating a new survey in Connect. Just use the default questions/template and publish that. Just try uploading a blank word document as your Feature Report. Do you receive the same error? Cool, Portal's working.
  2. Re-publish the old XLSX under a new name. Do you receive the same error? If not, original publish was corrupted.
    1. If you do receive the same error, try copy-pasting in segments and publish one block at a time. See where it breaks.

Thanks, apparently when I appended data to the HFS it wouldn't let me update the template, although the exiting one still worked.

Can you update the template after refreshing the page? If so, then that would be expected behaviour, as I mentioned above.

I was able to refresh after deleting all the records.  Apparently the template update scans the data in some way.
