GIS Request Management Solution - empty "Submited by", "Email", etc. fields in Activity Dashboard tables

Our Activities tables in both our Activities and My Activities Dashboard do not display field values for Email, Submitted By, Requested By, Full Name, Name, etc. Also some fields show up in duplicates.

I can see the values in the related GIS_Activities_surveys feature layer (hosted view).

Did we mess up our configuration?

There is information there regarding adding the fields to the table.  Not only do you need to add the field on the "Data" tab, but you must also go to the "Values" tab.

Clarification: To trouble shoot, I appended a "_" to the label of fields with duplicate labels in the Visualization configuration.
