If I enable an extension in ArcGIS Online and I currently have ArcGIS Pro open (which is the most typical use case), is there any way to "refresh" my licensing within ArcGIS Pro to activate the extension? Or is the only way to activate the extension to close and re-open ArcGIS Pro?

If it's the latter, I will probably put this into ArcGIS Ideas. Enabling and disabling extensions with a concurrent license server is quick and easy with the Enable and Disable extensions scripts. Now that we are all pretty close to only being able to use named user licenses, it would be good to have a way to enable an extension without closing and re-opening the application.

Restarting is the way to update the licensing.
