I got this message. It is a template geoform from ArcGIS Online I used. I checked web map with layers and it is working fine but geoform is no longer working. Please assist.  Thank you.

Can you share the Geoform publicly? I am being prompted to login  

My first suggestion would be to look at the network traffic with Dev tools and/or Fiddler

Sloppy mistake. Needed to take the prefix of 'cityofdoral.maps' off the URL.

It looks like the form is looking for an item with the ID of '3D216e45de70284a929a34f49310917694'

Can you search that ID in your AGOL org?
