Showing attachments from a secured related table not possible in Map Viewer?

Without direct reference to what you currently have, can only offer a basic pop-up view of one of our inspections.
The pop-up shows the asset information then, as can be seen below, the latest inspection and then the option to view a list of all.

Clicking on the latest record views the related inspection and all images (we have a criteria of 3 images per fortnightly asset inspection)

Again not sure of your workflow, however, we've got around 120 images per single asset and, from what I can see, following the process above will just ruin pop-up performance. I suppose there would be a use if you wanted just the latest single image, but not sure how much of a benefit that would really be.

Showing attachments from a secured related table is supported in Map Viewer. There are a couple of different ways to set that up. 

The article that you linked to above may have thrown you off since it outlines a specific workflow using an arcade expression to fetch the first attachment and bring it directly into the pop-up for the layer. 

Another option is to add the related record element to the feature layer pop-up. That allows you to quickly view information like the number of related records, and navigate through them all, where you can view their attachments (as seen in the previous comment). 

This article will walk you through setting that up in just a couple of steps. In your case, you'll need to be sure to configure the pop up for the related table first to display the attachments.
