Printed map has no legend

Our ArcGIS Online maps that are exported to PDF versions used to have legends on them, but no longer do. 

While looking for answers, I stumbled across this example code from the ArcGIS API for JavaScript, and noticed that their example map does not contain a legend on the printed version either:

The closest other thread along these lines is one in which the user was missing some (but not all) icons in the legend:

I would greatly appreciate any help in resolving this.

We've been experiencing the same issues with our Javascript Webmap Applications as well. Since about version 3.3 or 3.4 of the Javascript API the legend that was appearing on our PDF printed maps has completely disappeared.

Stranger still, even if we print using the default Webmap Applications provided by ArcGIS Online, we still don't get a map legend.

What happened to our map legend.

I think they changed it up and put it in the 'configure app' part of the Web Mapping Application. I see it way down at the bottom and when checked I can see it on the PDF printout.

Has this been updated in ArcGIS Online Web App builder? I can't seem to get the legend to print with the out of box print service.
