I have an ESRI global and ArcGIS online "free version" account that I use with the same user name and pwd, do I have to create a separate account for the 30 day "subscription" trial version with a new user name and password?

Yes, you do. If you choose to subscribe at the end of the trial, then you'll be able to bring in your current personal account.
