Basic Viewer Template

I followed the steps posted here:

I tried it dozens of times with the same data/MXD/service etc. and only once I was able to see the 'Configure App' button and use the application configuration page. All other times this option didn't show on the page at all. All I could see are the usual buttons: open, share, edit, delete, move to and attach code.
An I followed the instructions above and clicked the 'go to the item now' but the 'Configure App' button never showed up.

Suggestions?

I am unable to reproduce your problem. What browser are you using? Can you give me a set of steps you're doing so that I may reproduce the problem?

Note, not every template is configurable. Only those that have a gear icon over it are. The rest, are not configurable. I've circled the configurable applications in the attached image.
