When I post data to ARCGIS.com (a CSV file) where is that data then stored? Who has access to use/duplicate/see that data if I do not share it and make it public?

When uploading the CSV file, the data is stored in "the cloud", meaning, the data is hosted in a server environment for you.  If you choose to not share it with anyone, you will be the only person with access to the data.  When you do decide to share it, you have options to share it with your organization, certain groups, or everyone.

  Assuming the user never shares the data is the data permanently deleted when the user deletes the data.

Yes.  If the user deletes the data, there is no way of re-obtaining it.  They would need to add the CSV file to ArcGIS Online again.
