Query widget export to CSV only exports FID

I have a query widget operating and if I go to a result and choose to export query results to a CSV, the CSV only contains the FID for that layer and none of the other configured fields. So far I've found it is only the query widget that does this, whereas the Table widget and the Select widgets both export all fields in the CSV. How can I enable the query widget to also export other fields and not only the FID?

Seems ExB doesn't allow you to only allow export for one widget, it's an all or nothing for each feature so I cannot just disable export for query and force users to push query results to the table for exporting to other formats.

Unable to reproduce the problem. Can you provide a sample app?

Data was shared from ArcGIS Pro to our Portal (Enterprise 11.3) as a Map Image layer. So there are multiple layers in the Map Image service. Perhaps you can reproduce with that info? I cannot share a sample app due to sensitive data.
