Getting error 404 with Forms in ArcGIS Enterprise Map Viewer.

Hello all,
We are facing the below issue with forms in Web Map viewer. We are trying to get the related data editing to work following the below reference:

But, when we try to open this form, we are getting this error 404.
We have ArcGIS Enterprise 11.3.
