Is there any way to just be able to download a particular form using Survey123 template in App Studio for arcgis?

I am basically trying to use a Survey123  template  and I just want my user to have a access to one particular form to download? How do i achieve it? Highly appreciate the help.

I have this same question i have been struggling for months! no one can answer...they only tell you how to change the colors in your form, but not how to have just your form open on startup. If you have any answers, please let me know! I am also looking to pay a developer if I can find one!!
