Tiled map service not properly showing

Can someone help me out with a few problems I am having with a tiled map service? I have a cached tiled map service that I have as my basemap layer. I also have this same basemap layer service put under operational layers so that it will display in my layer list widget. First off, it is not showing up in my layer list widget, and I have tried setting it as both dynamic and tiled (although it seems like it needs to be set to tiled because it is a cached map) and I have also tried setting it to visible="true" and visible="false".

The other problem I am having is that at one particular scale, 1:16,000....the first 1/4 of the county..from the top of the county to the bottom, is just blank....nothing showing up?! I have deleted the cache and re-cached the service....tried clearing the rest cache in admin, however, nothing seems to do the trick. When I run HttpFox....I am getting "333 GET  500 text/html (NS_ERROR_FAILURE)" errors?? They are all for the basemap/mpaserver/tile/4/xxxx/xxxx...what does this mean and how can I fix it?

It might help if you read my post "Help formatting config.xml to make FlexViewer show map appropriately." I have pictures of what it looks like and a picture of the HttpFox errors as well. I would really appreciate insight on this problem and the ones in that post as well. Its pretty repetitive to waht I have posted here. I would love to have this web app done by tomorrow because it is the last day of my internship. I know I have asked many questions and been helped tremendously by all the ESRI staff in this forum as well as many of other users in the forum and I am extremely grateful for eveyone's help. You all have helped me learn a great deal about Flex as well as Flash Builder. If I could just get these little quirks resolved I would be one happy camper:) Thanks again for all those who have contributed so much of their time and work resolving past issues and I hope to hear back throughout the day on these last concerns.

Awesome, once I go closer than 1:4000.....nothing shows up...the screen is just black for my last two scales, 1:2000 and 1:1000. WHAT IS GOING ON???
