Can anyone help me resolve the error that i'm getting when i am trying to submit the Survey Form to ArcGIS Online?

Hi Guys,

This is the screenshot of the error that i'm getting while submitting the survey form to ArcGIS Online. I've used so many nvarchar values 'Yes'. I haven't been able to find which one is causing the error. (I have attached the xls form too)

Hi Imran,

You've forced a lot of the select_one questions to be of fieldtype 'esriFieldTypeInteger' when they should be 'esriFieldTypeString' or left empty - this is what was causing the error in the app,

You might want to also look at your choices sheet and the Name coloumn. A lot of the answers contain spaces or characters not accepted by Survey123 and some of the answers exceed 32 characters.
