Hi Daniel,
In your XLSForm, you have some fields using the same name repeatly. That's why you are getting the 400 error.
Please try to name each field with unique name and publiush should work just fine.
Thanks for the help. I renamed by columns so they all have unique names (I think) but it still is giving me the error code. Attached is the updated xls form screenshot. Thanks for the help.
