Please, I need help in publishing my survey. I have tried several times it keeps saying unable to add feature service definition. Code: 400 index was outside the bounds of the array. Attach image was the displaying information .

To be able to assist we need more information like are you publishing to ArcGIS Online or Portal for ArcGIS as part of ArcGIS Enterprise? Are you publishing a new survey or updating an existing one? What version of Survey123 Connect? Do you have issues with publishing other surveys, or from different devices or apps like ArcGIS Pro?
Can you provide the XLSForm?
Otherwise technical support will be able to search their database for cases with this error message, specific to survey123
