Esri Support here-- I haven't heard of others with this exact issue. But I have some follow up questions to help isolate in on the source of the problem: 1) Do you see the issue with all surveys? If you publish a new simple test survey, can you open that one? (This test would then let you know if the issue is with existing surveys, or all surveys) 2) Do you see the issue on multiple devices? 3) Are your surveys published to Portal for ArcGIS or ArcGIS Online? I'd check to see if your survey items are still available in "My Content" either way. I hope this helps get you started in finding out where the issue is coming from.

iOS is really particular about case sensitivity. I would check to see if the file names are identical with case sensitivity. Ex: picture.jpg vs. Picture.jpg In my experience, this makes a difference with iOS

Are you working with hosted feature layers in ArcGIS Online? If so, you could create a hosted feature layer view off of your main hosted feature layer used in Survey123. From the view you can define an expression to filter out features, allowing you to then only show a subset of the data when you export it. You can do this with the following steps: 1) Go to the item details page of the hosted feature layer 2) From the righthand side of the page, click "Create View" > make the view 3) Go to the item details page of the hosted feature layer view > Click the "Visualization" tab 4) Click the ... icon in the table of contents menu 5) Click "Set View Definition" > "Define Features" > Here you can filter based on your attributes of interest 6) Then, when you export the csv for this hosted feature layer, it will only include your filtered features based on what you defined in the view definition in Step 5

So it sounds like the issue is when you visualize the data in ArcGIS Online after publishing the survey. Am I correct in that? If so, when you add the hosted feature service to the web map you can configure the popups such that the 1,000's separator is removed. That workflow is detailed here (easiest just to do a CTRL + F for "1,000"): Configure pop-ups—ArcGIS Online Help | ArcGIS However, another workflow you could look into is setting the field to be a text field and then using a constraint to not allow letters. The way I set this up required a bit of creativity. In the constraint column, I used ".>1000000000" (thereby assuming that all phone numbers will be greater than this, 100-000-0000). So entering A999999999 would not work, because it is still not greater than 100-000-0000). Then to avoid confusion, you can just add something to the constraint message column for that field prompting them to enter only numbers. Hope that helps!
