Public survey showing "cannot access in web app"

Hello -

I am not able to share surveys or features publicly on my portal. It says that it cannot be accessed or cannot access web app (web form). I have it set to public and anonymous user turned on. Any ideas that could fix this. Any help is appreciated. Thanks!
