JS Error on Survey123 Web Form

Question

When I submit my web form, I receive the following error message: "Oak Grove Open Burning Permit Application

Unable to get property 'forEach' of undefined or null reference." The data is not going through.

Does anyone know if there is a way I can fix this on my end or if it is something wrong with the web form?

Hi Rachel,

Is this happening across all browsers, or only one?  Is this the survey that I helped you previously with on images?  If so, I appear to have just sent successfully.
